An approved PARI Image Request Agreement Form allows access to the PARI Images database for one calendar year. Access will be issued after a completed agreement is received and approved by PARI Respiratory Equipment, Inc. Thoroughly read and understand the binding terms of this agreement.
Once approved, a unique username and password will be issued to the contact email provided on the agreement. Please allow 24 to 48 hours for receipt of login information. The sharing of this login information is strictly prohibited. The approved Image Request Agreement will remain on file and valid for a period of one calendar year from signed date. A new agreement is required at the end of this term to continue access and use of files.
